Also, the organization cannot have a national or international affiliation, unless there is a local chapter serving the needs of Lake County. Use this sheet to ensure all pertinent information is presented. It is not required to use this form, but all the listed questions must be addressed in the presentation.

1.     Are they a recognized 501(c)(3)? (Note: This is a REQUIREMENT for consideration)

2.     What is the name of the organization?

3.     Where is the organization located? (Note: Organizations must serve Lake County)

4.     What is the mission of the organization?

5.     What purpose does this organizations serve? (Who? What do they do? How do they do it?)

6.     How would the organization benefit from receiving the donation? How will the funds be used?

7.     Who in the candidate organization is responsible for discharging the funds?

8.     What percentage of the donation will go to administrative and marketing fees? (Hopefully, none!)
